FMEA (Failure Mode and Effects Analysis) - definition

FMEA (Failure Mode and Effects Analysis) is a methodical analysis technique used in quality engineering and risk management to identify potential failure modes (failures) of systems, products or processes and assess their impact on the functioning of the whole. The main objective of FMEA is to systematically identify how a component or process may fail, what effects this may have and what preventive or corrective actions can be taken to reduce the risk of such events occurring.

PFMEA (Process Failure Mode and Effects Analysis) is a particular variation of FMEA, focusing on the analysis of manufacturing and operational processes. Unlike the classic product-oriented FMEA, the PFMEA examines the risks associated with the performance of technological, logistical and assembly activities, taking into account process variables, machine and operator capability, environmental conditions and human factors.

The process of conducting a PFMEA includes identifying possible causes of defects, assessing their severity (Severity), probability of occurrence (Occurrence) and possibility of detection (Detection), which leads to the calculation of the Risk Priority Number (RPN). High RPN values indicate areas requiring corrective action or process improvement.

The use of FMEA and PFMEA enables organisations to minimise costs associated with product defects, improve product quality and reliability, increase customer satisfaction and meet the requirements of quality standards such as IATF 16949, ISO 9001 or aerospace and medical industry standards. In modern quality management systems, the FMEA technique is often supported by IT tools that enable dynamic updating of analyses and integration with PLM and MES systems.
